CSA Reporting Form

The first campus official identified as Campus Security Authority (CSA) who receives information about a Clery reportable incident or disclosure, must file a CSA Report as soon as possible after the incident or disclosure is received. Any CSA receiving information about the same incident or disclosure should file a CSA Report even if one has already been filed. CSA's are not reponsible for determining whether a crime took place.
